When completing the form, please check the “Change Request” box on the first page of the form.

 

The completed form must be received by March 15, 2019 to ensure the change will be implemented before the 2019-2020 FY Ongoing Support Payments are issued in April 2019.

Update for January 10, 2019

 

The Minister of Health has announced a new financial support program for eligible Canadian Thalidomide Survivors. The new program is expected to begin in spring of 2019 and the application process will remain open for five years.

 

Survivors already supported by the TSCP will automatically transfer to the new program and will continue to receive their benefits uninterrupted.

 

The Administrator does not have any additional information about the new program at this time; however, you are welcome to contact the Administrator to request updates about the new program by calling 1-877-507-7706 (TTY 1-877-627-7027) or by email at tscp-pcst@crawco.ca.

Update for January 1, 2019

 

The annual Outreach Newsletter will be issued in February via your preferred method of communication (email or mail).  If you receive correspondence by email, please ensure that tscp-psct@crawco.ca is added to you address book.

 

Following the newsletter, we will also send you a brief survey to complete. Your comments about the TSCP program help us identify areas in the process where adjustments may be beneficial. You will be able to complete the survey on-line, via email, by phone or by mail.

Update for December 1, 2018

 

Survivors who are currently assessed at Disability Level 1 or 2 may request reassessment of their health status once per year. If you believe that your health has deteriorated since your last assessment, you can request reassessment by completing the Reassessment form available on the Forms page of this website or you may request the form be sent to you by mail or email by calling, writing or emailing the Administrator. Please complete the form in its entirety, as detailed answers will help the medical assessor to better understand you as a person and how the degeneration of your health has impacted your life and your ability to live independently as you age.

 

The Extraordinary Medical Assistance Fund  is also available to assist you with your extraordinary medical expenses and/or home or vehicle adaptation related needs. A schedule of possible eligible expenses is available on the website or by calling or emailing the Administrator.  If you are not sure if an expense may be eligible, we suggest that you submit an application anyway for consideration. You may submit a maximum of two EMAF applications per fiscal year. EMAF application forms are available for download on the Forms page of this website or you may also request the form be mailed or emailed to you by contacting the Administrator by phone, email or mail.

Additional pages can be used to convey important information to the assessor that you think they should know that may not be specifically addressed in the application.

Update for July 1, 2018

 

The Extraordinary Medical Assistance Fund (“EMAF”) has been modified to ensure that the program continues to meet the needs of Survivors.  For the 2018-2019 fiscal year, you may submit your application anytime between April 1, 2018 and March 31, 2019.  The application will be reviewed and payment will issued (if assessed to be eligible) at time of submission on a first come first served basis.

 

To request EMAF funding, Survivors must complete an EMAF application in full and submit photocopies of receipts for expenses already incurred or quotations for expenses that you would like covered by the EMAF. Each receipt/quotation should contain a clear description of the expense incurred or to be incurred.

 

Survivors may continue to submit a maximum of two EMAF applications per fiscal year unless there is urgent need, which will be evaluated on a case by case basis.

Update for June 1, 2018

 

To help us better understand the impact that the Thalidomide Survivors Contribution Program (“TSCP”) is having on Survivors, we would appreciate if you would please take a few minutes to fill out the annual survey to provide us with your valuable feedback about the TSCP, the status of your health and the service you have received to date. If you have not received the survey, please contact us.

 

You may now complete the survey directly from your electronic device (computer, mobile phone or tablet).  If you received your survey via mail and would like to fill out on an electronic device instead, please call or email the Administrator to request a link to the online survey.

 

You may complete and submit the hard copy by mail to the address indicated in the survey or you may call the Administrator at 1-877-507-7706, 1-877-627-7027 (TTY) to complete the survey verbally.

Update for May 1, 2018

 

The Administrator recently distributed the annual Outreach newsletter. If you have not received the newsletter, please contact us. As indicated in the Outreach newsletter, a brief survey will be sent to you this month. We request that you complete the survey to let us know how the Thalidomide Survivors Contribution Program is working and meeting your needs.

Update for April 1, 2018

 

Ongoing Support Payments for the 2018-2019 FY will be issued on or about April 30, 2018.  If you wish to change how you are receiving your payment, please complete and submit a new Ongoing Support Payment form and check the “Change Requested” box on the first page of the form.  Changes will take effect starting with the next Ongoing Support Payment following approval of the form once submitted. To have any changes apply to the 2018-2019 FY, please submit your form by April 10th at the latest.

 

Reassessment may be requested once per year between April 1st of the current year and March 31st of the next year. Any increase to a Survivor’s Ongoing Support Payment amount as a result of change in disability level would take effect April 1st of the next fiscal year.

 

The Extraordinary Medical Assistance Fund (“EMAF”) has been modified to ensure that the program continues to meet the needs of Survivors. A letter with attachments was sent recently to all Confirmed Survivors explaining the changes. The website has also been updated to reflect those changes. If you do not see the changes on the website, please refresh your Internet browser.

Update for March 5, 2018

 

We are aware that on February 27, 2018, Budget 2018 announced that the TSCP would be expanded to address the possibility that some thalidomide survivors may have been excluded by the current eligibility criteria.

 

While we do not have any additional information, Budget 2018 indicated that further details may be announced in the spring. We expect that this announcement will also indicate when claimants can begin to apply to the Program.

 

Please note that although the Government has announced an intent to change the Program, there are several processes that must take place before individuals can begin to apply.  We appreciate your patience.

 

If this is the first time you are learning about the TSCP and are interested in the Program, you are encouraged to contact Crawford & Company (Canada) Inc., the independent third-party administrator, directly at 1-877-507-7706 (TTY 1-877-627-7027) or by email at TSCP-PCST@crawco.ca to provide your contact information. While the administrator does not have additional information about the Budget 2018 announcement or the anticipated program changes, your contact details can be retained to inform you once additional information becomes available.

 

Claimants may check back periodically and are requested to keep Crawford informed of any changes to their contact information.

Update for February 1, 2018

 

Ongoing Support Payment:

 

To request a change in regard to how you will receive your annual Ongoing Support Payment for the upcoming year, please ensure your completed Ongoing Support Payment form is received by the Administrator no later than March 15, 2018. The form is available for download from the website or by calling or emailing the Administrator to request one be sent to you. When completing the form, please check the “Change Requested” box on the first page of the form and ensure that you include all of the required supporting documents including a copy of your identification.

 

When choosing between yearly or monthly payments, please note that in the event of the death of a Survivor after the payment process begins, the Survivor’s Estate will be permitted to keep the payment received in the month that the Survivor passed away if the Survivor chose to receive payments monthly. Thereafter Survivor payments would stop to the Estate. If a Survivor chose to receive a lump sum annual amount, the Estate will be permitted to keep the full amount for that year regardless of the date of death of the Survivor. Then Survivor payments would stop.

Update for November 1, 2017

In response to feedback received from Survivors, we will not be issuing a fall Outreach Survey. The Outreach Survey will now be produced on an annual basis in the spring of each year. The next survey will be sent in the spring of 2018.

 

The second deadline to apply for the EMAF for the 2017-2018 Fiscal year is postmarked by January 15, 2018.

 

The deadline to apply for Reassessment is postmarked March 31, 2018. Update for October 1, 2017

 

The first deadline for the EMAF has passed. The applications submitted well before the deadline have been reviewed. We also received several applications at the deadline and are in the process of reviewing those. We anticipate sending out decision letters by the end of this week unless additional information is needed from survivors who submitted their applications at the deadline. Because there is a fixed amount of money in the EMAF per year, the Administrator must review all of the applications submitted by the deadline before funds may be allocated to each individual survivor.

 

If all applications are complete, payments will follow within the next two weeks. Please be aware that if the funds requested is greater than the funds available, not everyone who submitted an application may receive funding and/or individuals may only receive a portion of the amount requested on a pro-rata basis.

Your feedback is very important to the Administrator as it helps us to identify areas in the process where adjustments may be beneficial to help the program run as smoothly and efficiently as possible.

Update for June 1, 2017

 

Survivors have access to an Extraordinary Medical Assistance Fund (“EMAF”) twice per fiscal year which was created to help survivors cover costs relating to their extraordinary health support needs such as specialized surgeries and home or vehicle adaptations.

 

Survivors are able to submit requests to the EMAF to help cover the cost of home or vehicle adaptations even if the province or territory in which he or she resides has a similar program to assist with these expenses. As such, if a Survivor has applied to a provincial or territorial program for help with the cost of an extraordinary home or vehicle adaptation and are waiting to learn if it will be covered, the Survivor can choose to submit his or her claim to the EMAF for consideration instead.

 

The EMAF is not intended to cover ongoing expenses such as routine dental cleanings/scaling/checkups, expenses due to the normal aging process, ongoing medications or attendant care required on a continual basis. The cost of medically necessary insured hospital or physician services is also not covered because this is prohibited under the Canada Health Act.

 

If you are unsure whether or not an expense is eligible, please submit it anyway and the Administrator will review the request and determine whether or not that expense is extraordinary or specialized. If you feel a “routine” expense is extraordinary please provide additional details to support your request.

 

The distribution of the EMAF funding at each deadline in a fiscal year depends upon the total volume of applications received and the total of all expenses submitted for that period.

The deadline to apply for the EMAF for the 2016/2017 Fiscal Year is postmarked by May 15, 2017. You may only submit receipts for expenses incurred between April 1, 2016 and March 31, 2017. There will be no carry over of expenses already purchased into the EMAF for the 2017/2018 FY.

Update for March 1, 2017

The Administrator has received inquiries regarding income tax implications with the payments provided through the Thalidomide Survivors Contribution Program.

 

We have received confirmation from the Canada Revenue Agency that any annual lifetime payments and payments from the Extraordinary Medical Assistance Fund (EMAF) are not taxable under the Income Tax Act.

 

Please be advised that the Administrator has sent Survivors via email, a copy of the correspondence from the Canada Revenue Agency.

When requesting reassessment, providing complete information and detailed responses to the question in the Health Assessment section of the application is very important.  The information you provide helps the medical assessor who is reviewing your file better understand you as a person and how the degeneration of your health over time has impacted your quality of life and your ability to live independently as you are aging.

 

Although optional, we highly recommend submitting photographs with your reassessment.  The photographs may be helpful to the medical assessor to better understand the nature and extent of your physical injuries when information contained within the medical documentation is minimal or absent.

Update for January 1, 2017

 

Thank you to everyone who completed and returned our fall Outreach survey.  Your feedback is very important to us and help us identify areas in the process where adjustments may be beneficial to help the program run as smoothly and efficiently as possible.  From the responses received we are aware that you would like more information on the Ongoing Support Payment process.

 

Prior to the Ongoing Support Payments are issued to you, the Administrator awaits the release of funds from Health Canada.  Once the funds are received, our finance department then issues the payments by your preferred method. While every effort is made to receive and issue the payments on or about the 30th of each month sometimes this is not possible due to for example statutory holidays. Generally if you have chosen to receive your payment as monthly installments, payments are issued on or about the 30th of each month.  Depending upon your banking institution, direct deposits should appear in your account within 1-3 business days.  Individuals who chose to receive payments by cheque, should receive their payment within 5-10 business days of the 30th depending upon your place of residency.

If you missed the November 15, 2016 deadline to submit an EMAF (Extraordinary Medical Assistance Fund) Application, you still have the opportunity to apply for 2016-2017 fiscal year. The next deadline is May 15, 2017.  The application must be postmarked in the province or territory where you reside by the deadline date.

Update for September 21, 2016

 

The Administrator distributes an Outreach newsletter twice per year to provide Survivors with updates about the Thalidomide Survivors Contribution Program: once in the fall and once in the spring. The fall edition of the Outreach newsletter will be distributed at the beginning of October.

 

In this edition of the newsletter, you will find information on upcoming important dates, ongoing support payments, medical assessments, the EMAF, and privacy.  You will also be given an update on the previous Outreach survey results.

 

Two to three weeks following the newsletter, the Administrator will be sending you a brief survey to complete.

 

Now that the program is underway, updates will be posted once per month of the first of the month. The next status update will be November 1, 2016.

 

 

Update for July 20, 2016

 

The Reassessment Process:

 

When requesting reassessment, providing complete information and detailed responses to the questions in the Health Assessment section of the application is very important. The answers and responses you provide help the medical assessor who is reviewing your file better understand you as a person and how the degeneration of your health over time has impacted your quality of life and your ability to live independently as you are aging. Additional pages can be used to convey important information to the assessor that you think they should know that may not be specifically addressed in the application.

 

As well, submitting photographs of your injuries when requesting reassessment, although optional, may also be helpful to the medical assessor to better understand the nature and extent of your physical injuries when information contained within medical documentation is minimal or absent.

 

Lastly, you are not limited to submitting medical documentation from a family practitioner solely. We suggest that you submit documentation from the medical professional(s) who best understands the nature and extent of your injuries and how those injuries impact your quality of life.

 

All of the above will aid in you receiving the most comprehensive assessment possible.

Qualification Applications – Deadline was May 31, 2016. Late application forms will not be accepted.

 

Reassessment Applications – The deadline to qualify for a retroactive top up payment at a higher level for the 2015/2016 fiscal year was May 31, 2016.  Reassessment Application are still being accepted for the 2016/2017 fiscal year and onward. Please be advised that survivors may request reassessment once per year or more urgently on a case by case basis.

 

EMAF Applications – The deadline for 2015/2016 EMAF Applications was June 15, 2016.  EMAF Applications are still being accepted for the 2016/2017 fiscal year which has two deadline dates: postmarked by November 15, 2016 and May 15, 2017.

 

Assessment Applications – The deadline to submit the Assessment Application is postmarked by July 15, 2016.

Update for June 8, 2016

The Administrator has recently distributed the first Thalidomide Survivors Contribution Program Newsletter.  The purpose of this newsletter is to provide Survivors with information and updates relating to the program.  The Administrator will distribute the newsletter twice per year: once in the spring and once in the fall.

Following each newsletter, the Administrator will also contact survivors by telephone within a few weeks of the newsletter`s distribution.  This is to obtain valuable feedback from each survivor about the program, the status of the survivor`s health and to confirm the survivor`s contact and ongoing support payment method of delivery information.

Update for May 11, 2016:

 

Confirmed Canadian Thalidomide Survivors have access to an Extraordinary Medical Assistance Fund (“EMAF”) which was created to help survivors cover costs relating to their extraordinary health support needs such as specialized surgeries and home or vehicle adaptations. The EMAF can only provide support for such things that are not otherwise covered by provincial/territorial healthcare plans or by other service providers to respect equal access to insured services.

 

To apply for funding under the EMAF, you may contact the Administrator to request that an EMAF application be sent to you or you may also download an EMAF application from the website. The EMAF application will be available shortly.

 

The review of EMAF applications and the process to distribute funding will depend upon the volume of applications received and the cost of expenses submitted for that period. The Administrator will complete its evaluations and distribute the funds in as fair a manner as possible.

 

EMAF applications will be reviewed and payments will be issued twice per year unless a request for more urgent help is made. Urgent requests will be reviewed on a case by case basis to decide if the request will be granted.

 

The EMAF aspect of the Thalidomide Survivors Contribution Program is a component which may evolve over time as the needs of the survivors change and provincial/ territorial plans evolve. The Administrator will be collecting information from Survivors through an outreach survey, conducted twice per year, to gain a greater understanding of the needs of Survivors as they age.

Update for April 27, 2016:

 

The 2016-2017 Fiscal Year Ongoing Support Payments will be issued within the next week.

 

Similarly, if you elected to receive your 2015-2016 Ongoing Support Payment in monthly installments, the April installment will also be issued within the next week.

 

Each Confirmed Survivor will receive a letter by mail confirming that payment has been issued.

Update for April 13, 2016:

 

Qualification Application – Documentary Proof:

 

When completing Section 4 of the Qualification Application, if you are having difficulty providing proof of maternal ingestion of Thalidomide under the brand names Kevadon or Talimol in Canada during the first trimester of pregnancy, the following may be considered acceptable documentary proof:

 

  1. Copy of a doctor's prescription indicating that Thalidomide was prescribed for the individuals’ mother during the first trimester of pregnancy; or,

  2. Hospital birth records of the individual making reference to his/her mother’s ingestion of Thalidomide during pregnancy; or,

  3. Other medical/pharmacy records indicating that the individual’s mother was prescribed/ingested Thalidomide during pregnancy; or,

  4. If no records are available, proof in the form of a sworn affidavit from a medical professional with direct knowledge of the event may be acceptable, e.g. physician stating that he/she prescribed Thalidomide to the individual's mother during pregnancy. IMPORTANT: An affidavit from a non-medical professional (e.g., mother/father, relative, friend, etc.) would not be accepted. For a list of individuals who may swear an affidavit, see below:

Update for March 30, 2016:

 

The 2015-2016 Fiscal Year Ongoing Support Payments have been issued.

 

Payments have been issued following the instructions of the Survivor in the Ongoing Support Payments form. If no form was submitted, then payment has been issued by cheque.

 

Each Confirmed Survivor will receive a letter by mail confirming that payment has been issued.

Update for February 17, 2016:

 

Confirmed Canadian Thalidomide Survivors will receive their initial ongoing support payments as follows:

 

For the 2015-2016 fiscal year – payment will be issued on/or before March 31, 2016.

 

For the 2016-2017 fiscal year – payment will be issued beginning in April 2016. This payment will be increased by 2% (for inflation).

 

Retroactive top ups due to a change in disability level, as a result of reassessment, will be issued in September of 2016 for both the 2015-2016 and 2016-2017 fiscal years.

 

Claimants who the Administrator qualifies as eligible Canadian Thalidomide Survivors will receive their initial ongoing support payments as follows:

 

For the 2015-2016 fiscal year – payment will be issued on/or before September, 2016.

 

For the 2016-2017 fiscal year – payment will be issued on/or before September, 2016. This payment will be increased by 2% (for inflation).

Update for February 5, 2016:

 

Application forms are being distributed. If you haven't received your form within 2 weeks, please contact the Administrator.
